their identity. Numerous public and private sector agencies and
companies have information that can assist you in aspects of
identity theft. If your concern relates to the following categories,
contact the agency directly for help and information. Follow the
steps on the individual web pages to get your information.
Government Agencies
Department of Motor Vehicles
If your driver's license has been lost or stolen, you must notify
your local Department of Motor Vehicles. Inform your DMV of your
situation, request a fraud alert and file a complaint form to begin
a fraud investigation. It is also important to notify your DMV if
your state uses your SSN as your driver's license number. Request
that they use another client identifier number besides your SSN.

Contacting the IRS concerning Tax concerns.
Call 1800-829-0433 or www.treas.gov/irs/ci. You should also call
the Internal Revenue Service Taxpayer Advocates Office at 1-877-777-4778.
The IRS is in charge of handling all criminal incidents regarding
tax laws.
Contacting the U.S. Postal Service
Notify the U.S. Postal Inspector if your mail has been stolen or
tampered with. See the U.S. Postal Inspection Service in your telephone
listing under Federal Government or www.usps.gov/websites/depart/inspect.
Contacting the Social Security Administration
Contact the Social Security Administration's Fraud Hotline at 1-800-269-0271
if you believe someone is using your SSN to apply for a job or to
work. To request a personal earnings report contact 1-800-772-1213
or www.saa.gov.
Contacting the Federal Communications Commission

contact the Federal Communications Commission (FCC). Contact information
for the FCC is 1-888-CALL-FCC or file complaints on line at www.fcc.gov
or fccinfo@fcc.gov.
Contacting the Department of State regarding Passports
To contact the Passport office or the United States Department of State log on
to www.travel.state.gov/passport.
Contacting the Securities and Exchange Commission regarding Investments
If you believe that someone has accessed your securities investments
or a brokerage account, immediately report it to your broker or account
manager and to the Securities Exchange Commission (SEC). File a complaint
with the SEC by logging on www.sec.gov/complaint.shtml. You can also
contact them at 202-942-7040.
Contacting the U.S. government regarding Bankruptcy
See the appropriate government agency in your telephone listing under
Federal Government listings or write to the U.S. trustee in the regional
area or go to ww.usdoj.gov/ust or U.S. Trustee offices.
Federal Bureau of Investigation
The FBI is charged with protecting the U.S. from terrorist attacks,
from foreign intelligence operations, and from cyber-based attacks
and high-technology crimes; combating public corruption at all levels;
protecting civil rights; combating international and national organized
crime, major white-collar crime, and significant violent crime; supporting
our law enforcement and intelligence partners; and upgrading FBI technology.
The FBI will investigate fraud-related matters that cross state lines
and the US border. Log on the FBI website at www.fbi.gov.
Federal Trade Commission
Contact the Federal Trade Commission's ID Theft Clearing House to
file a complaint. The FTC was charged by the United States Congress
to take complaints form those whose identities have been stolen. Victims
of identity theft can call 1-877-438-4338 to or www.ftc.gov to report
your incident. The FTC places your information into a secure consumer
fraud database and may in some cases be shared with law enforcement
agencies and private entities in resolving identity theft complaints.
